The Role
A superb opportunity for a motivated and passionate Clinical Pharmacist to join an established GP surgery.
You’ll work closely with GPs, ANPs, nurses, and pharmacy technicians to improve patient safety, health outcomes, and the quality of prescribing. You’ll benefit from mentorship with a GP and Lead Pharmacist, and gain access to NHS England’s CPPE Primary Care Pharmacy Education Pathway, supporting you to practice and prescribe safely within a primary care setting.
Key Responsibilities:
- Conduct face-to-face and telephone medication reviews, including long-term condition management (e.g. diabetes, hypertension, heart failure)
- Provide expert medicines advice to patients and clinical staff
- Manage medicines reconciliation following hospital discharge and new patient registration
- Support safe, evidence-based, cost-effective prescribing in line with NHS and ICB guidance
- Implement national prescribing guidance and MHRA alerts
- Lead or contribute to quality improvement and audit projects
- Provide education and training to the wider primary care team
- Contribute to CQC compliance, public health initiatives, and clinical governance
About You
Essential:
- GPhC-registered Pharmacist
- Undertaking or exempt from the CPPE Primary Care Pharmacy Education Programme
- Working towards or willing to complete an Independent Prescribing qualification
- Excellent clinical, communication, and decision-making skills
- Passionate about improving patient outcomes and medicines optimisation
Desirable:
- Previous experience within a GP practice or primary care setting
- Membership of the Primary Care Pharmacy Association (PCPA) or Royal Pharmaceutical Society (RPS)
