Job Responsibilities

  • Provide a Personal Advisor service as required by relevant acts and regulations.

  • Offer advice and support to young people in various categories to help them transition to independent living.

  • Co-work with social workers and case manage young people in specific categories.

  • Supervised by the 16 Plus Senior Practitioner and managed by the Team Manager within the 16 Plus Service.

  • Promote independence by supporting young people with budgeting and purchasing items for their property.

  • Complete and review Pathway Plans and My Pathway Planner documents with young people.

  • Promote the wishes and feelings of young people and ensure their needs are met.

  • Coordinate services as identified in the Pathway Plan.

  • Develop effective working relationships with internal and external colleagues.

  • Respond flexibly to crisis situations and maintain regular contact with young people.

  • Arrange access to translation, interpreter, and advocacy services as needed.

  • Maintain accurate case recording and use the Social Services database.

  • Contribute to the overall effectiveness of the team and participate in duty rotas.

  • Pursue personal development and attend appropriate training.

  • Promote the Authority’s Equal Opportunities Policy.

  • Work with other professionals and outside organizations to support young people.

Person Specification

  • Qualifications: NVQ Level 2 in Child Care or equivalent Level 2 qualification. NVQ Level 3 Caring for Children and Young People.

  • Knowledge: Understanding of relevant acts and regulations, working with children and young people, and IT literacy.

  • Skills: Effective communication, teamwork, conflict resolution, and ability to work in a dynamic environment.

  • Experience: Working with children, young people, and families in complex situations and crisis management.

  • Other: Full UK Category B driving license, ability to work flexibly, and commitment to confidentiality and data management.
