A leading specialist care service in Hitchin is looking for an Assistant Practitioner to join their dedicated team. This service supports adults living with a range of neurological conditions, providing personalised care that promotes independence, dignity, and well-being.
You’ll play a vital role in supporting a high standard of care within a multi-disciplinary team, working under the guidance of registered professionals including nurses and therapists.
The Role
As an Assistant Practitioner, you will:
- Support the assessment, planning, and delivery of person-centred care
- Monitor and report changes in service users' conditions
- Communicate effectively with colleagues, residents, and families
- Help maintain a safe and supportive environment
- Uphold service user choice, dignity, and participation
- Encourage a positive, respectful team culture and challenge unsafe practices when needed
What You’ll Need
- Level 5 Foundation Degree in Assistant Practitioner (Healthcare)
- Experience working in healthcare, ideally in neurology or palliative care
- Familiarity with clinical guidelines and relevant legislation
- Experience working within a multi-disciplinary team
- Excellent communication and interpersonal skills
- Compassionate, responsive, and professional approach
