An exciting opportunity is available for an experienced Band 7 Community Physiotherapist to join a dynamic team in Hounslow. This full-time, ongoing locum role offers the flexibility and variety of community-based work, alongside a competitive hourly rate of £30.00. You will play a vital role in supporting patient recovery following discharge and promoting long-term wellbeing in a community setting.

• Conduct comprehensive assessments and deliver targeted interventions for patients post-discharge.

• Manage triage of referrals from both discharge hubs and the Home First initiative.

• Handle telephone referrals, ensuring accurate communication and appropriate follow-up actions.

• Collaborate effectively with a multidisciplinary team to ensure high standards of patient care.

• Develop and implement individualised care plans tailored to patients’ specific rehabilitation needs.

• HCPC registration as a qualified Physiotherapist is essential.

• Must be a car driver, with the ability to travel between community locations as required.
