Are you an experienced Senior Social Worker looking for a new challenge? We have an exciting opportunity for you to join a Hospital Discharge Team on the Isle of Man, helping patients transition safely from hospital to community care.
The Role:

Facilitating safe and effective hospital discharges

Conducting assessments and coordinating care plans

Working within a multi-disciplinary team to ensure the best outcomes

Supporting patients and their families through the discharge process

Managing complex cases and safeguarding responsibilities

Requirements:

Degree in Social Work & Social Work England registration
