This service provides supported accommodation for young people with high support needs, helping them regain stability, reach their full potential, and achieve independence.Key Details

  • Caseload: Support and guide a caseload of 6 clients, working closely with them to address their individual needs and aspirations.
  • Service Type: A supported accommodation project for 16 young people (aged 16-25) with high support needs.

Responsibilities

  • Provide housing management support to ensure a safe and stable living environment for clients.
  • Offer guidance and signposting to help clients access employment, education, and training opportunities.
  • Address health and wellbeing concerns by connecting clients with necessary services and resources.
  • Arrange and conduct 1-1 key work sessions to promote independent living skills, goal setting, and emotional support.
