Pay Rate: Up to 38 p/h
Job Description:
Join our Community Social Work Team (CSWT) in Kingshill, Kent, and help provide essential support to vulnerable individuals and families in the community.
Key Responsibilities:
- Conduct comprehensive assessments to determine care needs.
- Develop and implement care plans, ensuring they meet the needs of service users.
- Provide ongoing support to individuals, helping them achieve independence.
- Ensure that safeguarding and risk management procedures are followed.
- Work alongside healthcare professionals and agencies to ensure seamless service delivery.
Qualifications & Requirements:
- Essential: Social Work qualification (e.g., Degree in Social Work, Postgraduate Diploma in Social Work)
- Essential: Registration with Social Work England (SWE).
- Previous experience in a similar role, with a strong understanding of community-based social work.
- Excellent interpersonal and communication skills.
- Strong time management and organizational skills.
