Job Title: Social Worker - Mental Health - Kingston Upon ThamesLocation: Kingston Upon ThamesPay Rate: £ 31.31/per hour
Liquid Personnel is currently seeking an experienced Adult Social Worker to join the Mental Health Team of our esteemed client. We are looking for a qualified social worker with experience in mental health to review supported living placements. The majority of these reviews will be conducted jointly with Health partners.
Responsibilities :
- Evaluate patients' mental health status, identifying needs, strengths, and areas for intervention.
- Collaborate with the healthcare team to create individualized treatment plans that address patients' mental health needs and goals.
- Respond to mental health crises, providing immediate support and intervention to stabilize patients.
- Attend interdisciplinary team meetings and case conferences to discuss patient care and collaborate with other healthcare professionals.

Qualifications and Experience : To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
