This is a vital role, ensuring adults at risk receive timely and appropriate support.About the Role:
- Conduct thorough assessments of vulnerable adults, identifying their needs and risks.
- Develop and implement person-centered care plans, coordinating services and resources to meet individual needs.
- Work collaboratively with multi-agency partners (health, housing, voluntary organizations) to ensure holistic support.
- Advocate for vulnerable adults, ensuring their voices are heard and their rights are protected.
- Maintain detailed and accurate records, adhering to statutory guidelines and timescales.
- Contribute to a supportive and collaborative team environment focused on continuous improvement.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience in adult social care is essential; experience with vulnerable adults is highly desirable.
- Excellent assessment, communication, and case management skills.
- Strong understanding of relevant legislation and policies relating to adult social care.
- Ability to work independently and as part of a team.
- Commitment to person-centered practice and ethical standards.
