Social Worker - Assessing Vulnerable Adults Team (Dudley Council)Make a positive impact on the lives of vulnerable adults in your community.Dudley Council is seeking a compassionate and experienced Social Worker to join our dedicated Assessing Vulnerable Adults team. This is a vital role, ensuring adults at risk receive timely and appropriate support.About the Role:

  • Conduct thorough assessments of vulnerable adults, identifying their needs and risks.
  • Develop and implement person-centered care plans, coordinating services and resources to meet individual needs.
  • Work collaboratively with multi-agency partners (health, housing, voluntary organizations) to ensure holistic support.
  • Advocate for vulnerable adults, ensuring their voices are heard and their rights are protected.
  • Maintain detailed and accurate records, adhering to statutory guidelines and timescales.
  • Contribute to a supportive and collaborative team environment focused on continuous improvement.

Requirements:

  • Registered with Social Care England.
  • Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience in adult social care is essential; experience with vulnerable adults is highly desirable.
  • Excellent assessment, communication, and case management skills.
  • Strong understanding of relevant legislation and policies relating to adult social care.
  • Ability to work independently and as part of a team.
  • Commitment to person-centered practice and ethical standards.
