Job Advert - Social Worker (Older People Team) - Surrey

Surrey County Council is seeking an experienced Social Worker to join their Older People Team, delivering a high-quality professional service to adults, their families, and carers in line with the Care Act.

The role involves assessing needs, promoting independence, and supporting individuals to remain safe and well in the community. You will use an asset-based approach and work closely with families, carers, and multidisciplinary teams to develop outcome-focused support plans. The post requires strong professional judgment, risk management skills, and the ability to support individuals through changing, complex circumstances.

Key Responsibilities:

Complete Care Act assessments and identify eligible needs for adults and carers.

Develop, review, and monitor person-centred support plans promoting choice, control, and independence.

Encourage involvement and ownership of care planning, including clear explanation of Direct Payments.

Support individuals to retain skills, confidence, and prevent or delay deterioration.

Work with families to facilitate contingency planning.

Consider NHS Continuing Healthcare as part of assessment work.

Support the duty/intake function by responding to urgent issues and managing risk.

Provide escalation and professional guidance to colleagues where required.

Apply and uphold key legislation, including the Care Act, Mental Capacity Act, Human Rights Act, and safeguarding responsibilities.

Support Surrey's duties around Counter Terrorism and the reduction of radicalisation risks.

Ideal Candidate:

Qualified Social Worker registered with SWE.

Experience in Adult Social Care, preferably with older adults.

Strong assessment, safeguarding, and risk management skills.

Confident working in a strengths-based, person-centred way.

Able to work collaboratively with families, carers, MDTs, and partner agencies.
