About the Role:
- Provide high-quality support and care to adults with learning disabilities, promoting their independence and well-being.
- Collaborate with individuals, their families, and other professionals to develop and implement person-centered care plans.
- Conduct thorough assessments to identify needs and support individuals in achieving their goals and aspirations.
- Advocate for individuals' rights and access to appropriate services and support.
- Contribute to a positive and supportive team environment focused on client-centered care.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a Social Worker.
- Experience working with adults with learning disabilities is highly desirable.
- Excellent communication, interpersonal, and case management skills.
- Commitment to person-centered care and ethical practice.
