Qualified Social Worker, Older People’s TeamPay rate £38 per hour Vitalis are recruiting for a Qualified Social Worker to work in an Older People’s Team in Lincolnshire.2 Social workers needed to cover the South area of Lincolnshire. Office base is flexible such as Sleaford, Spalding, Grantham, Stamford and be able to work across area teams when needed. Social workers need to have experience in using Mosaic, undertake adult needs assessments and know what to care plan and commission. Extensive experience in working with adults with physical disabilities and adult frailty older persons. Extensive experience in completing Mental capacity assessments and strength-based approaches and knowledge, An understanding of signposting and Direct payments. as well as experience of CHC and taking part in DST's.
