Social Worker - Assessing Social Worker Team (Lincolnshire County Council)

Lincolnshire County Council is seeking a highly skilled and experienced Social Worker to join our dedicated Assessing Social Worker Team. This is a vital role, ensuring vulnerable adults receive appropriate and timely support.

About the Role:

Conduct comprehensive needs assessments of adults, identifying their strengths, needs, and risks.
Develop person-centered care plans, ensuring individuals receive the right support in the right place.
Work collaboratively with individuals, their families, carers, and other agencies to coordinate care and resources.
Advocate for the rights and needs of vulnerable adults, promoting their independence and well-being.
Manage a diverse caseload, balancing competing priorities and ensuring timely interventions.
Contribute to the ongoing development and improvement of assessment practices within the team.


Requirements:

Registered with Social Care England.
Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience in conducting thorough needs assessments for adults is highly desirable.
Excellent assessment, communication, and case management skills.
Strong understanding of relevant legislation and policies relating to adult social care.
Ability to build rapport and trust with individuals and their families.
