Job Description:
Key Responsibilities:
• Complete strength based assessments for people with disabilities within the authority’s eligibility criteria, applying the Care Act 2014 and other relevant legislation.• Establish partnerships with people with disabilities and carers (including young carers), actively using consultation and participation.• To recommend, demonstrate and organise the supply of equipment and structural adaptations to meet the assessed need.• To work collaboratively with other professionals, statutory, voluntary, community resources and other stakeholders to ensure the best possible services are provided for people and their carers within available resources.• Manage a diverse workload within Adult Social Care priorities and authority’s policies. • Utilise appropriate training and development opportunities, participating in your own professional development in line with criteria set out by the Allied Health Professionals authority.
