Liverpool City Council is looking for agency social workers for its Adult Mental Health multi-disciplinary team!
Fantastic pay rate of £46.08 per hour!!
Details & Responsbilities of the role:
- Attend ward reviews to work as part of mental health multi-disciplinary teams in adults and older persons wards
- Complete assessments of need under the Care Act and develop strengths based support plans.
- Key in developing discharge plans and lead on these
- Complete Mental Capacity Assessments and make Best Interest decisions when required
- Partnership working with health, housing, children’s services etc essential
Requirements of the role:
- Qualified Social Worker
- Fully Social Work England registered
- Must have significant post-qualified experience in adult social care
- Must have Mental Health experience
