A new and exciting locum opportunity has arisen within a fast paced Hospital discharge team in the Liverpool area.

You will be joining the Adult Hospital Discharge Team helping to provide a high quality of service to all service users in the areas of the Adult Discharge. The team supports the wards to facilitate safe and timely discharges of patients from the hospital, working within the Discharge to Assess model.

Responsibilities:
  • You will carry out assessments under the Care Act 2014 to determine eligibility need
  • Maximise rehabilitation services such as reablement & intermediate care where appropriate
  • Provide assessment and appropriate interventions to aid patients in achieving optimum recovery/rehabilitation and quality of life.
  • Work with patients and families addressing the impact of illness and treatment
To be considered for the Social Worker role:
  • 1-3 years experience of working within a similar hospital discharge setting
  • Diploma/Degree in Social Work
  • SW England registration
  • Hospital experience is an advantage
