This role is based within an Adult Social Care Hospital Pathway Team. The successful canddiate will be reporting to the Manager. The role will involve:

Being responsible for leadership and management of social care team. Using their advanced knowledge and experience to lead and model practice which meets statutory requirements, provides early help and prevention to reduce need and support independence using the relevant practice framework for the service. Managing complex case loads, safeguarding enquiries, court of protection applications, continuing health care assessments, working jointly with other professionals. Supervision of qualified and unqualified staff members.
