What You'll Do:
- Conduct assessments and create personalized care plans for patients
- Visit patients in their homes, communities, or hospitals
- Collaborate with our care team to ensure the best outcomes for patients
- Provide ongoing support and supervise care packages
- Participate in reviews and multi-sign-offs (with extra compensation)
What We Need:
- A qualified Registered Nurse
- Strong assessment and care planning skills
- Excellent communication and teamwork abilities
- Flexibility to travel between locations
- Ability to work independently
