EHC Coordinator Role - £307 Per day Please see responsibilities and duites below.

  • To manage the whole assessment and placement process for Children and Young People with SEND to a high degree of quality, ensuring that assessment processes are completed within statutory deadlines.
  • To draft proposed Education, Health and Care Plans (EHCPs) identifying key aspects of the professional advice and summarising in line with the SEN Code of Practice, legislation and Council policy.
  • To contribute to required planning for the 14+ transition process.
  • To prepare case briefings for SEND Panels in order to enable the decision making process.
  • To attend meetings as a representative of SEND & Integrated Service, either in own right or in the absence of the Team Manager.
  • To be responsible for seeking SEND placements at appropriate schools and providers and facilitate these placements.
  • To monitor placements and attend annual and transition reviews, as appropriate ensuring that the pupil's needs are met and the Council receives value for money. In particular, cases where placements breakdown, to take responsibility for involving other professionals and securing an alternative placement. This may include acting as Lead Professional/Practitioner.
  • Organise and attend meetings with other professionals concerning Children and Young People with SEND, including annual and transition reviews as appropriate.
  • To maintain up to date records of pupils with SEND providing an audit trial of involvement from which statistical information will be produced, as required.
  • To read and evaluate annual, transition review reports and other requests for funding from schools parents or settings and be responsible for making a recommendation to SEND Panel, Extended Panel, High Needs Panel and Tripartite Panel, as appropriate.
  • To liaise with other local authorities in relation to placements in Merton schools where the Children or Young People have EHCPs issued by another authority.
  • To liaise with other local authorities where a pupil with a Merton EHCP needs to be placed in an out of borough school.
  • To support the implementation of the SEND & Inclusion Strategy, as agreed with the Team Manager.
  • To liaise with and advise parents/carers, schools, colleges, providers and other stakeholders on aspects of the SEND Code of Practice, SEND and related legislation as it affects the conduct of statutory assessments and annual reviews in relation to individual children and young people.
  • To ensure and maintain effective communication with key statutory and voluntary sector partners. This could include attendance at case conferences, planning meetings and children protection case conferences.
