This is an excellent opportunity to make a real difference by supporting individuals discharged from acute hospitals into short-term beds and helping them transition back home safely and independently.  JOB DESCRIPTIONThe duties and responsibilities of the role will include:
  • Complete occupational therapy assessments for individuals discharged into short-term beds to enable timely and safe discharge home.
  • Undertake person-centred assessments and promote individuals' health, wellbeing, and independence.
  • Work collaboratively with individuals with complex needs to develop support plans that empower choice and maximise independence.
  • Design and implement interventions to reduce the impact of disability and ill health
THE SUCCESSFUL CANDIDATEThe successful candidate will have the following skills and competencies:
  • A recognised Diploma or Degree in Occupational Therapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Strong understanding of relevant Health and Social Care legislation, policies, and current practices.
  • Demonstrable experience within adult social care and hospital discharge planning.
