This is an excellent opportunity to make a real difference by supporting individuals discharged from acute hospitals into short-term beds and helping them transition back home safely and independently. JOB DESCRIPTIONThe duties and responsibilities of the role will include:

Complete occupational therapy assessments for individuals discharged into short-term beds to enable timely and safe discharge home.
Undertake person-centred assessments and promote individuals' health, wellbeing, and independence.
Work collaboratively with individuals with complex needs to develop support plans that empower choice and maximise independence.
Design and implement interventions to reduce the impact of disability and ill health

THE SUCCESSFUL CANDIDATEThe successful candidate will have the following skills and competencies:

A recognised Diploma or Degree in Occupational Therapy.
Registration with the Health and Care Professions Council (HCPC).
Strong understanding of relevant Health and Social Care legislation, policies, and current practices.
Demonstrable experience within adult social care and hospital discharge planning.
