Job Overview:We are seeking an experienced Band 6 Occupational Therapist to join our community team, working in Marlow and Thame. This role focuses on elderly care, conducting community visits, and delivering person-centered therapy to enhance patients' quality of life and independence.

Key Responsibilities:

  • Manage a community-based caseload, focusing on elderly care and rehabilitation.

  • Conduct home assessments and implement interventions to improve safety and independence.

  • Collaborate with multidisciplinary teams to coordinate comprehensive care plans.

  • Provide advice and support to patients, families, and carers to achieve therapy goals.

  • Document patient progress and maintain accurate records in line with trust policies.

Key Requirements:

  • HCPC registration as an Occupational Therapist.

  • Proven NHS experience in community and elderly care settings.

  • Knowledge of current practices in community rehabilitation and home safety adaptations.

  • Strong communication, organizational, and teamwork skills.

  • Access to a vehicle and a valid driving license for community visits.
