About the Role:
Join the Hospital Team as an Adult Social Worker, conducting Care Act assessments, support planning, and leading Safeguarding Enquiries while collaborating with health, housing, and voluntary organisations to ensure comprehensive care for vulnerable adults.
What will your responsibilities be?
In this role, you will support Southwark's @Home/Urgent pathway, a hospital admission avoidance service. You will intervene with residents who can be supported home from hospital emergency departments (ED) and those at risk of admission from home. Your responsibilities include assessing patients' needs, coordinating care plans, and ensuring a safe transition back to their homes. You will collaborate with healthcare professionals to provide comprehensive support, aiming to reduce unnecessary hospital admissions and improve patient outcomes.

Essential Requirements:
- Social Work England registration
- Eligibility to work in the UK
- Full UK driving license
- Post-qualified experience
- Degree level or equivalent in Social Work
