Key Info:
2 Social workers needed to cover the South area of London. Office base is flexible and be able to work across area teams when needed. Social workers need to have experience in using Mosaic, undertake adult needs assessments and know what to care plan and commission. Extensive experience in working with Adults with physical disabilities and Adult frailty older persons. Extensive experience in completing Mental capacity assessments and strength based approaches and knowledge, An understanding of signposting and Direct payments. as well as experience of CHC and taking part in DST's. Also adults safeguarding experience is needed.
Requirements & Qualifications:
To be successful in this role, you will need:
- a current and valid Social Work England registration and pin
- experience working within adults community settings
- strong communication and interpersonal skills
- the ability to work independently and within a multidisciplinary team
- a commitment to delivering high-quality patient care
- access to a car for community/home visits
Working Hours & Contract Duration:
This role offers a flexible working position requires availability from Monday - Friday with flexibility where possible. The contract is for 3 to 6 months with a strong possibility of extension based on performance and service needs.
