You'll be joining a proactive and supportive Access and Wellbeing Team focused on prevention, enablement, and strength-based support. This includes medium to longer-term social work interventions, safeguarding enquiries, reassessments, and reviews.

?? Must be available to be physically present in the office 3 days per week?? Role includes:
- Home visits �?
- Joint working with other professionals ??
- Duty responsibilities ??
? About You:
- Qualified and progressed Social Worker, registered with Social Work England.
- Completing Care Act assessments ??
- Court of Protection and Human Rights assessments ??
- Undertaking Carers Assessments
