Key Info:

The Social Worker in the Crisis team will work alongside health colleagues to identify and address the social determinants of the crisis, supporting service users to access services and benefits, supporting their carers and enabling them to maintain independence in the community as far as possible. To use individual and family interventions to effect change and work co-productively with service users in reaching agreed outcomes. 

The role will be for a hospital discharge social worker and they will need to be literate in Mosaic and Rio and used to working within a statutory setting and cmht.

Requirements & Qualifications:

To be successful in this role, you will need:

• a current and valid Social Work England registration and pin
• experience working within adults mental health teams
• strong communication and interpersonal skills
• the ability to work independently and within a multidisciplinary team
• a commitment to delivering high-quality patient care 
• access to a car for community/home visits
• previous hospital discharge, CMHT and Crisis experience ideally

Working Hours & Contract Duration:

This role offers a flexible working position requires availability from Monday - Friday with flexibility where possible. The contract is for 3 to 6 months with a strong possibility of extension based on performance and service needs.
