The responsibilities of the Social Worker includes:
- Carry out complex needs assessments for adults and their carers.
- Create, implement, and review person-centered care and support plans.
- Manage a complex caseload with a high degree of autonomy.
- Lead complex safeguarding inquiries and multi-agency risk assessments.
- Ensure full compliance with all relevant legislation, including the Care Act 2014 and Mental Capacity Act.
- Work collaboratively with a wide range of health and social care professionals.
- Advocate for service users and their families.
- Provide advice and support to clients and their network.
- Maintain accurate and timely case records and reports
The ideal candidate will have:
- Social Work England registration
- Eligibility to work in the UK
- Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
