We are looking to recruit a Qualified Social Worker in the Proactive Frailty and Complex Care Team (PROFACCT) Team within Islington. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The responsibilities of the Social Worker includes:
  • A collaborative service between Islington Council and Whittington Health. This innovative team focuses on anticipatory care, working with adults living with frailty and complex needs to reduce hospital admissions and promote independence
  • Deliver strengths-based assessments and interventions under the Care Act, Mental Capacity Act, and other relevant legislation.
  • Work collaboratively with residents, carers, community organisations, and health professionals.
  • Attend and contribute to multi-disciplinary team (MDT) meetings.
  • Identify and respond to safeguarding concerns using a Making Safeguarding Personal approach.
  • Support anticipatory care planning and early intervention to prevent escalation of need.
  • Promote the values of CARE (Collaborative, Ambitious, Resourceful, Empowering) and iCARE (Innovation, Compassion, Accountability, Respect, Excellence).

The ideal candidate will have:
  • Social Work England registration
  • Eligibility to work in the UK
  • Social Work qualified (MA/BSc/DipSW/CQSW/MSc/BA)
