Ackerman Pierce is currently supporting a leading local authority within Lewisham to recruit for a Senior Social Worker in the Hospital Discharge Team within the Social Care industry. This will be on a permanent basis paying 48,618-50,646 per annum.
Responsibilities include:
- Working with Vulnerable Adults within a hospital setting
- Report writing and making assessments
- Developing care plans
- Care management
- Managing complex cases
- Liaising with other professional bodies
- Arranging safe and timely discharges for service users
- Self-directed support / personal budgets
- Supervise Social Workers and Case Management Officers
- Handle complex cases, including MCA Assessments, Court of Protection applications, and s42 safeguarding enquiries
Experience preferred:
- Post qualifying experience of working as a Senior Social Worker - Hospital Discharge team within the UK Social Care industry.
Minimum Requirements:
- HCPC Registration
- 1 year post qualifying experience within UK Social Care industry
- Enhanced DBS/CRB (we can obtain for you)
- Eligibility to work in the UK
