Location: Kings College Hospital, Denmark Hill, SE5 Team: Transfer of Care - South (Older Adults & Physical Disabilities) Rate: £32 per hour (umbrella) Hours: Full-time ( Duration: Ongoing Registration: MUST be Social Work England registered About the Role: We are seeking an experienced Locum Social Worker to join Southwark's Transfer of Care Team (TOC South), based at Kings College Hospital, to support safe, timely and person-centred discharges for adults with complex needs. Working within Adult Social Care (Older Adults & Physical Disabilities), this is a hospital-based role where you will carry out complex discharge planning, including Mental Capacity Assessments, safeguarding, and multi-agency working. Key Responsibilities: Deliver high-quality, strengths-based social work under Southwark's practice framework Lead complex casework, including safeguarding and risk management Undertake timely assessments of need and capacity (including MCA) on the hospital wards Coordinate safe and sustainable discharge plans with NHS, therapy teams, care providers, and families Support and mentor less experienced team members and students Engage in collaborative, multidisciplinary discharge planning meetings Work Pattern: This is not a remote post due to the need for ward-based assessments and in-person discharge planning. 3-4 days/week required on-site (Jenni Lee House, SE5 - walking distance from Kings College Hospital) 1 day/week working from home for administrative tasks is usually acceptable Requirements: Social Work England registration (essential) Experience in hospital discharge/TOC or similar adult social care teams Sound understanding of MCA, safeguarding, and the discharge process Ability to manage complex caseloads and work collaboratively with multi-agency teams Strong written and verbal communication skills Why Join Us?
