Role: High Risk Complex Case Coordinator

Connect2Luton are excited to recruit a High Risk Complex Case Coordinator on behalf of Luton Borough Council within their Adult Social Care department.
Main purpose of position:
The post is a full time, 2-year grant funded position to support the Local Authority to provide multi-agency interventions in the high risk adult vulnerability in Luton, coordinated through monthly multi-agency meetings.
As a High Risk Complex Case Coordinator,you will be responsible to:
- To effectively chair, organise and facilitate the High Risk Complex Case Panel monthly meeting in a timely manner, ensuring a suitable venue is booked and that members are informed of dates and times.
- Organising and chairing professional meetings for various High Risk Complex Case subjects as required, leading case conference meetings where you will initiate.
- To develop and maintain effective working relationships with colleagues, across the Council and with multi agency partners to ensure a fully integrated.
- Be responsible for maintaining a working relationship with key partners.
- To assist the team manage casework systems and processes, and professionally produce all relevant casework documentation.
Skills and Experience
- Post Social work qualifying experience including in-depth experience of: complex case work with different adult client groups, advocacy and user and carer involvement issues, business/ financial management, taking effective action to achieve performance improvements.
- Good English language and writing skills.
- Ability to set up, develop and maintain partnership operational processes enabling staff to maximise service delivery.
- Effective communication, negotiating, influencing skills, gaining commitment from a wide range of partners.
- Ability to work both collaboratively and independently with little supervision.
- Training and experience working with Adults.

Because of the nature of this job, it will be necessary for Disclosure and Barring Service checks to be undertaken.
