Job Scope:

We are looking to recruit a committed and organised Care Coordinator to join a dedicated home care team known for delivering high-quality, person-centred support. As a Care Coordinator, you will play a key role in ensuring smooth and efficient care delivery, adapting to changing schedules, supporting care staff, and maintaining strong communication with clients and their families. Working closely with the Registered Manager, the Care Coordinator will oversee day-to-day scheduling using digital care planning systems, ensure compliance with CQC standards, and help maintain the services excellent reputation for quality and reliability.

This is a dynamic role where the Care Coordinator will ensure healthcare workers receive timely supervisions, training, and support, while also managing client feedback and contributing to continuous improvement. With centralised teams providing HR, payroll, and IT support, youll be free to focus on what matters most - ensuring the right care is delivered at the right time. This role includes participation in the on-call rota and offers genuine opportunities for career growth.

Do you have?

  • A strong commitment to delivering high-quality, person-centred care and promoting individual wellbeing
  • Excellent organisational and time management abilities, with a proactive approach to workload prioritisation
  • Knowledge of CQC regulatory standards and compliance requirements within the care sector
  • Effective communication and coordination skills, with the ability to lead and support care staff professionally
  • A recognised qualification in Health and Social Care (such as NVQ) is highly desirable but not essential
