TheCareManagerwilltakeownershipoftransitions,start-ups,andtheongoingmanagementofcarepackages,workingcloselywithfamiliesandmulti-disciplinaryteamstodeliverthehigheststandardsofsupport.Youwillregularlyreviewcareplansandriskassessments,maintainingfullcompliancewithsafeguardingandregulatorystandards.
