About the Role:Rotherham Council is looking for a dedicated and experienced Hospital Social Workerto join our fast-paced hospital discharge team. You’ll play a vital role in supporting adults during their transition from hospital to home or other care settings, ensuring safe and timely discharges.

Key Responsibilities:

  • Carry out strength-based assessments and discharge planning

  • Work closely with NHS colleagues, hospital staff, and families to coordinate care

  • Ensure discharges are in line with current legislation, policies, and best practice

  • Promote independence and person-centred approaches

  • Maintain clear, concise records in line with Rotherham Council’s standards

Requirements:

  • Qualified Social Worker with active Social Work England registration
