About the Role:Rotherham Council is looking for a dedicated and experienced Hospital Social Workerto join our fast-paced hospital discharge team. You’ll play a vital role in supporting adults during their transition from hospital to home or other care settings, ensuring safe and timely discharges.

Key Responsibilities:

Carry out strength-based assessments and discharge planning
Work closely with NHS colleagues, hospital staff, and families to coordinate care
Ensure discharges are in line with current legislation, policies, and best practice
Promote independence and person-centred approaches
Maintain clear, concise records in line with Rotherham Council’s standards

Requirements:

Qualified Social Worker with active Social Work England registration
Previous experience in hospital social work or discharge planning
