Are you a dedicated and experienced Adult Social Worker passionate about supporting adults to achieve safe, timely, and effective discharges from hospital?

This is a rewarding and fast-paced role where you will be at the forefront of enabling adults to return to their communities with the right support, promoting their independence, well being, and ensuring a "Home First" approach wherever possible. You will work collaboratively within a multi-disciplinary environment, liaising closely with health colleagues, individuals, their families, and carers to deliver person-centred outcomes.

About the Role:

As an Adult Social Worker in our Hospital Discharge Team, your key responsibilities will include:

  • Proactively managing and coordinating the discharge process for adults with diverse and complex needs, including those requiring intermediate care or reablement services.
  • Conducting comprehensive, person-centred assessments of care and support needs, including undertaking Mental Capacity Act assessments and Best Interest decisions where appropriate, to inform robust discharge planning
  • Working collaboratively with individuals, their families, and multi-disciplinary partners to create, implement, and review effective, strengths-based discharge and support plans that promote independence and choice.
  • Actively exploring and implementing solutions that enable individuals to return to their own homes or familiar community settings with appropriate support.
  • Building and maintaining strong working relationships with health care professionals across acute trusts and community health services, as well as with housing providers, voluntary sector organisations, and internal council services.
  • Identifying and responding appropriately to safeguarding concerns, ensuring adherence to relevant legislation, local policies, and procedures.

About You:

We are looking for a resilient, adaptable, and enthusiastic professional who possesses:

  • A recognised Social Work qualification (e.g., Degree in Social Work, DipSW, CQSW, or equivalent).
  • Current registration with Social Work England.
  • Proven post-qualifying experience in adult social care, ideally within a hospital discharge, intermediate care, or other fast-paced, multi-disciplinary setting.
  • A strong understanding and practical application of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
  • Excellent assessment, analytical, risk management, and care planning skills.
