RoleRole & Responsibilities

You will conduct in-depth health assessments, develop and implement bespoke care plans, and coordinate care with a multi-disciplinary team. You'll empower patients and their families, monitor outcomes, and drive continuous improvement in care delivery. This leadership role offers the chance to make a tangible difference in complex care.

Main tasks include
  • Conducting in-depth health assessments.
  • Developing and implementing bespoke care plans.
  • Coordinating care with GPs, therapists, and social workers.
  • Ensuring the safety and effectiveness of care packages.
  • Contributing to local business growth and commissioning initiatives.
  • Empowering patients and their families.
  • Monitoring outcomes and driving continuous improvement.
Requirements
  • NMC Registered Nurse: Active PIN in Learning Disabilities Nursing or Children's Nursing.
  • Proven experience in complex care or related nursing roles.
  • Proactive, compassionate, highly organised, and a natural leader.
  • Excellent communication and interpersonal skills.
  • Strong analytical and problem-solving abilities.
  • Ability to work independently and as part of a team.
  • Commitment to providing high-quality patient care.
