This is a fantastic opportunity to lead, coordinate, and advocate for patients with complex care needs in a supportive and collaborative environment.The Role:

As a Complex Care Nurse Case Manager, you will be pivotal in designing and implementing person-centered care plans, coordinating services from multiple disciplines (GPs, therapists, social workers), overseeing care packages, and driving continuous improvement through data analysis. You'll also play a role in local business development and commissioning.Key Responsibilities:

  • Conduct comprehensive patient assessments
  • Develop and implement individualized care plans
  • Coordinate care across healthcare disciplines
  • Oversee care packages, ensuring safety and effectiveness
  • Advocate for patients rights and access to resources
  • Educate patients and families
  • Monitor patient progress and adapt care plans as needed
  • Collaborate with interdisciplinary teams
  • Analyze data to improve patient outcomes
  • Maintain accurate patient records
