ResponsibilityWhat will you be responsible for:

Coordinating comprehensive care for children with complex health conditions, ensuring they receive the right services.

Specifically the activities will be:

Assessment, care plan development, coordination with providers, patient advocacy, and excellent communication.

Experience in complex care management is a plus!
