Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services

Main Suties:

  • Manage a caseload as allocated
  • Undertake assessments under the Care Act and other legislation, using a strength- based approach
  • Co-ordinate and monitor care packages and support (care) plans and undertake reviews to ascertain whether the desired outcomes are being achieved. This may include advocating on behalf of service users or carers
  • Work with individuals to assist them to assess risk and take decisions related to their ability to function in their community environment
  • Lead Safeguarding Adults investigations using the local multi-agency policy and procedure (Level 2 Social Workers only)
  • Promote safeguarding awareness with service users and multi-disciplinary colleagues. Keep updated on current safeguarding guidance and practice and be vigilant in all activities
  • Take a lead in complex cases such as those with safeguarding issues, complex family dynamics, conflict and problematic situations (Under supervision for Level 1 Social Workers)
  • Provide written reports for court proceedings, case conferences and other arenas
  • Work with highly complex risk of all types, especially where there are capacity/best interest issues (under supervision for Level 1 Social Workers)
  • Engage actively with opportunities for inter-disciplinary relationships and partnership working and to encourage integrated ways of working wherever appropriate
  • Operate in accordance with NCC and departmental practice and guidance, including the maintenance of electronic social care records within Liquid Logic, and relevant departmental and corporate administrative procedures
  • Provide practice guidance to unqualified or less experienced staff under the supervision of the Team Manager/Practice Consultant (Level 2 Social Workers only)
  • Undertake, when appropriate the supervision of competency-based staff and/or students including learners from other agencies (Level 2 Social Workers only)
  • Ensure service delivery meets the ethnic, religious, cultural and linguistic backgrounds and to challenge appropriately where this is not the case
  • Maintain awareness of Departmental and other resources including specialist health resources and to be able to advocate for service users to access these appropriately
  • Encourage development of suitable resources and partnerships to benefit service users
