- Delivering short-term care to prevent hospital or care home admissions
- Supporting with personal care, medication prompts, and light meal preparation
- Providing end-of-life care with dignity and compassion
- Assessing client needs and creating tailored care plans
- Working closely with health and social care professionals
? What We’re Looking For
- NVQ Level 2 (or equivalent) in Health & Social Care
- Experience in delivering personal care and medication prompts
- Strong communication skills with a professional, empathetic approach
- IT literacy and good local area knowledge
- A full UK driving licence and access to a vehicle
- Flexibility to work varied hours
