Responsibilities
- Conduct assessments and interviews with individuals to gather information about their needs and circumstances.
- Develop and implement case plans to address clients' needs and goals.
- Provide counselling and support to clients, including crisis intervention and emotional support.
- Coordinate services and referrals to other agencies or professionals as needed.
- Monitor clients' progress and adjust case plans as necessary.
- Maintain accurate and up-to-date case records and documentation.
- Collaborate with other professionals, such as social workers, psychologists, and healthcare providers, to provide comprehensive care for clients.
Requirements
- NVQ 3 in Health & Social Care and/or relevant professional qualification.
- Experience of building productive working relationships with a range of partners.
- Experience of working in multidisciplinary and multi-agency teams.
- Experience of working towards achieving individual and team objectives.
- Knowledge of effective individual and group interventions to engage substance users and/or offenders in recovery focused treatment.
- Knowledge of the Substance Misuse Recovery Agenda effective substance misuse interventions and treatment options including the 12-Step Minnesota Model.
