About the Role:

If you are an experienced social worker, join the dynamic Response team, serving as the 'front door' for the adults care and support service. This is an exciting opportunity to make a real difference in the community.

What will your responsibilities be?

In this role, you will be responsible for triaging initial referrals into the service, offering information and advice and signposting individuals to alternative services such as the voluntary sector. You will complete Care Act assessments and create support plans, source care packages when required and triage Staffordshire Children Advice and Support Service (SCAS) referrals while undertaking safeguarding enquiries. Additionally, you will support community visits when necessary, ensuring that individuals receive the care and support they need to improve their quality of life. This position demands a proactive approach to managing referrals and providing comprehensive support to those in need.

Working Arrangements:

Qualifications & Experience:

To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, have at least 2 years of post-qualified experience in social work.
