We are currently recruiting on behalf of Wirral Council for an Experienced Adult Social Worker to join their team delivering Care Act assessments, reviews, and support planning. This role is ideal for a skilled practitioner confident in working with adults requiring support across a range of services including domiciliary care, supported living, 24-hour placements, extra care housing, and day services.
This is a great opportunity to work within a well-established team focused on delivering high-quality person-centred care and safeguarding vulnerable adults. If you have sound knowledge of the Care Act 2014 and Mental Capacity Act 2005, and experience with Continuing Health Care processes, this role offers a fulfilling and supportive environment to make a real difference.
Key Requirements:
Qualified Social Worker registered with Social Work England
Experience undertaking Care Act assessments and reviews
Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005
Able to conduct Mental Capacity Assessments (MCA) and Best Interest decisions
Sound safeguarding experience and skills
Experience with Continuing Health Care (CHC) and participation in CHC/MDT meetings
Willingness and ability to undertake face-to-face visits
Resilient, reliable, and committed to promoting independence and wellbeing
