Job Overview:We are seeking a dedicated Band 6 Physiotherapist to join the acute care and frailty response team at Whiston Hospital. This role supports both inpatient and community settings, aiming to provide early intervention and prevent unnecessary hospital admissions.

Responsibilities:

  • Deliver physiotherapy assessment and treatment for patients in acute and community frailty settings

  • Work within a multidisciplinary team to promote early discharge and prevent readmissions

  • Support complex discharge planning and deliver interventions at home when necessary

  • Contribute to service development and patient-centred care improvements

Requirements:

  • HCPC registration as a Physiotherapist

  • Minimum 6 months NHS experience, preferably in acute or frailty settings

  • Strong clinical reasoning and ability to work independently in the community

  • Car driver with access to a vehicle (essential for community visits)
