Eden Brown Synergy is recruiting an experienced Qualified Social Worker to join the Transition Team in Knowsley.
This role focuses on supporting young people aged 14-25, ensuring a smooth and well-planned transition from children's to adult services.

You will be responsible for planning and undertaking Care Act assessments, coordinating support, and ensuring appropriate services are in place as young people approach their 18th birthday and beyond.

A key focus will be on enabling young people to move successfully from education to community-based placements and into long-term, sustainable accommodation-supporting independence while maintaining wellbeing and safety.



Key Responsibilities

  • Undertake social care needs assessments (SCNA) to establish Care Act eligibility.

  • Develop, implement, and review support packages for young people.

  • Carry out Mental Capacity Act assessments and Best Interests decisions where appropriate.

  • Initiate Court of Protection/Deprivation of Liberty (CoP DoL) processes as required.

  • Work collaboratively with Children's Social Work Teams, Leaving Care, Adult Services, and Health colleagues (including Learning Disabilities and Mental Health teams).

  • Prepare young people and families for transition to adult services and ensure a smooth handover of care.

  • Promote a trauma-informed, person-centred approach and apply knowledge of transitional safeguarding and PBS/Active Support models.



Requirements

  • Qualified Social Worker (SWE registration required).

  • Proven experience working within transition services or similar settings.

  • Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures.

  • Experience using Liquid Logic case management systems.

  • Excellent communication, engagement, and time management skills.

  • Confidence to challenge decisions appropriately and seek clarification where needed.

  • Experience with CoP DoL processes, children's and adult safeguarding, and trauma-informed approaches is desirable.



Team & Induction

  • Office base days: Tuesdays and Fridays each week.

  • New starters are encouraged to work in the office full-time for the first two weeks to support induction and integration with the team.

  • You'll report to the Senior Practitioner and Service Manager and work collaboratively with a supportive, experienced team.
