We are seeking a dedicated Health Care Coordinator to join our team, supporting adults with diverse learning abilities. This role involves creating and implementing effective health care plans, promoting health, and providing training on health-related issues. The successful candidate will work closely with NHS and primary care services to enhance the health and well-being of the individuals we support.

Day-to-Day Responsibilities:
  • Develop and oversee effective health care plans in collaboration with the individuals we support and their care teams.
  • Facilitate access to health-related services and activities, acting as an advocate to challenge health inequalities.
  • Provide training and education on health-related issues to both individuals and their support teams.
  • Supervise health-related practices within the houses, ensuring all care is person-centred and meets individual needs.
  • Create health-related passports for areas such as meal support and incontinence support.
  • Maintain effective communication with individuals we support, their teams, and management.
  • Collaborate with the Healthy Living Team Leader and other health professionals to provide a holistic approach to health care.
  • Stay informed about new trends and services in the health care sector.
Required Skills & Qualifications:
  • Minimum of five years’ experience in health care or nursing.
  • Relevant professional qualification in health care or nursing.
  • Proven IT skills for creating Easy Read materials and health-related documents.
  • Experience working with individuals with diverse learning abilities.
  • Strong interpersonal and communication skills, with the ability to motivate and educate others.
  • Ability to plan, prioritise, and manage workload effectively in a demanding environment.
  • Flexible, team-oriented, and capable of handling various tasks as required.
