Job Summary:
As a Care Coordinator, you will be responsible for coordinating and managing the care of our complex care clients in the community. You will work closely with our team of healthcare professionals to ensure that our clients receive the best possible care and support.
Key Responsibilities:
Coordinate and manage the care of complex care clients in the communityDevelop and maintain care plans for clients based on their individual needsLiaise with healthcare professionals, families, and other stakeholders to ensure effective communication and coordination of careMonitor and review the progress of clients, making necessary adjustments to care plans as neededConduct regular assessments to identify any changes in clients' needs and update care plans accordinglyEnsure that all care is delivered in accordance with company policies, procedures, and regulatory requirementsProvide support and guidance to care staff, promoting a culture of continuous learning and developmentMaintain accurate and up-to-date records of client care and progressParticipate in on-call duties on a rotational basis, providing support and guidance to care staff outside of regular working hoursRequirements:
Minimum of 2 years' experience in a care coordination or management roleExperience working with complex care clientsKnowledge of relevant legislation and regulations in the healthcare sectorExcellent communication and interpersonal skillsStrong organizational and time-management skillsAbility to work well under pressure and handle multiple tasks simultaneouslyProficient in using computer systems and software for record-keeping and communication purposesWillingness to participate in on-call duties on a rotational basisIf you are a compassionate and dedicated individual with experience in complex care, we would love to hear from you.
