Job Summary:

As a Care Coordinator, you will be responsible for coordinating and managing the care of our complex care clients in the community. You will work closely with our team of healthcare professionals to ensure that our clients receive the best possible care and support.

Key Responsibilities:

  • Coordinate and manage the care of complex care clients in the community
  • Develop and maintain care plans for clients based on their individual needs
  • Liaise with healthcare professionals, families, and other stakeholders to ensure effective communication and coordination of care
  • Monitor and review the progress of clients, making necessary adjustments to care plans as needed
  • Conduct regular assessments to identify any changes in clients' needs and update care plans accordingly
  • Ensure that all care is delivered in accordance with company policies, procedures, and regulatory requirements
  • Provide support and guidance to care staff, promoting a culture of continuous learning and development
  • Maintain accurate and up-to-date records of client care and progress
  • Participate in on-call duties on a rotational basis, providing support and guidance to care staff outside of regular working hours
  • Requirements:

  • Minimum of 2 years' experience in a care coordination or management role
  • Experience working with complex care clients
  • Knowledge of relevant legislation and regulations in the healthcare sector
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Ability to work well under pressure and handle multiple tasks simultaneously
  • Proficient in using computer systems and software for record-keeping and communication purposes
  • Willingness to participate in on-call duties on a rotational basis
