Job Description:

Liquid Personnel is recruiting Social Worker for it client's Community Mental Health Team (CMHT) located at Rochdale.

What will your responsibilities be?

  • Undertake Strengths-Based Needs Assessments and review care packages for adults with severe and enduring mental health needs
  • Update care plans and complete Mental Capacity Assessments where required
  • Carry out safeguarding work in line with statutory duties when appropriate
  • Support timely hospital discharges for individuals under secondary mental health services
  • Work with both in-borough and out-of-borough placements; understanding of mental health pathways is beneficial

Qualifications & Experience:

To be successful in this role you must have,

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
