Connect2Halton is seeking an Early Help Family Worker to join Halton's Short Break Service for Children with Disabilities. The team consists of enthusiastic and dedicated workers who are committed to improving the lives of disabled children and their families.

The Short Break Service for Children with Disabilities Service works with families living in all areas of Halton and so you will have access to an office base in both Widnes, at Warrington Road Children Centre and in Runcorn, at Glendale Family Centre. You will be required to travel to visit families in their own homes and elsewhere in the community.

The role will involve regular travel across the Borough and sometimes further afield. Therefore, the post holder must have the ability to travel, sometimes at short notice, and be able to transport children and young people to interventions as required. Where necessary, reasonable adjustments will be made in accordance with the Equality Act.

Key Responsibilities

  • You must have a working knowledge of relevant government legislation, guidance and local procedures relating to disabled children, their carers and safeguarding.
  • You should have experience of working collaboratively with colleagues from other organisations, including social care, health, education, commissioned services and the voluntary sector.
  • Experience with children with disabilities 0-18 years old
  • Able to establish the needs of children with disabilities and supporting them to access short break services that will best meet those needs.
  • Complete MAP and Short Breaks assessments, coordinate services and implement & review action plans.
  • You will ensure that the voice of the child influences their support plan and will be working with other professionals to provide a holistic and person centred approach to supporting the child and their family.
  • You will manage an individual caseload.

Experience:

  • Experience working with children in a professional setting.
  • Strong communication and interpersonal skills to engage with families effectively.
  • Knowledge of child development and behavior management techniques.
  • Ability to work collaboratively with multi-disciplinary teams.
  • Understanding of safeguarding and child protection policies and procedures.
