Connect2Halton is currently recruiting for an experienced Qualified Social Worker to join the Short Break Service for Children with Disabilities Team within Halton Council. We are looking to appoint a specialist Social Worker who shares our commitment to provide services that enable families of children with disabilities to have an improved quality of life within their communities. You will have a key role in assessing the needs of disabled children who have complex needs, producing a robust, person centred, Child in Need plan and supporting them to access short break services.
Key Responsibilities:
- Manage a caseload of children and young people with disabilities in line with statutory requirements and departmental policies.
- Undertake assessments, care planning, and reviews to ensure individual needs are met effectively.
- Work in partnership with families, carers, education, health professionals, and other agencies to deliver coordinated support.
- Promote safeguarding and ensure all practice aligns with relevant legislation and guidance.
- Support children and families to access short break services that meet their needs and preferences.
Qualifications & Experience:
- Social Work England registration.
- 3 years perm experience.
- Previous experience of working with children with disabilities.
- Ability to work under pressure and manage caseloads.
